Bookings and inquiries

Sending the booking form is a request, not a confirmed appointment. Your schedule is confirmed only when our team contacts you.

Please give accurate and complete details, so we can prepare for your exam and reach you.

Medical results

Results are never released through this website. They are released only through the clinic’s official channels, to you or to a representative, employer or agency you have authorized.

Prices, packages and HMO coverage

Prices shown or quoted are subject to change until confirmed in writing. Corporate and agency packages follow the terms of the written agreement or quotation.

HMO coverage depends on your HMO’s approval and your plan. Please bring your HMO card and any required letter of authorization.

Certificates and credentials

Our licenses, accreditations and certificates are published so that clients can verify them. They may not be copied, altered or used to suggest that UNIMED endorses another party.

Content and trademarks

The UNIMED name and logo, and the text, photos and design of this website, belong to UNIMED or are used with permission. Please don’t reuse them without our written consent.

Acceptable use

You agree not to use this website to send false information, spam or harmful code, to try to gain unauthorized access to it, or for any unlawful purpose.

Links to other websites

This website links to services we don’t control, such as Google Maps and government agencies. We aren’t responsible for their content or privacy practices.

To the extent allowed by law, UNIMED is not liable for any loss arising from the use of this website or from it being temporarily unavailable.
